New, Transfer, or Closing Labs

New labs, building transfers, PI transfers, and closing labs should send notification through email to the Lab Safety Program (LabSafety@appstate.edu) or by completing this webform. An initial meeting will be held between relevant members of the EHS & EM office and the PI/department to discuss required actions. This discussion could include any or all of the following:

New Labs:

  • Suitability of lab space for proposed work
  • Lab Safety Program documentation (Lab Safety Plan, chemical/physical/biological SOP’s)
  • Applications to relevant safety committees on campus (IBC, IRB, IACUC, etc.)
  • Inspection schedule and frequency

Building Transfers:

  • Suitability of new lab space for proposed work
  • Transfer plan for hazardous materials between lab spaces
  • Lab Safety Program documentation updates (documents, door signs, emergency information)
  • Final disposition of research materials not being transferred

PI Transfers:

  • Transfer or application for relevant licenses
  • Lab Safety Program documentation updates (documents, door signs, emergency contact information)
  • Inspection schedule and frequency

Closing Labs:

  • Final disposition of research materials (biological, chemical, radiological, equipment, etc.)
  • Closure of any relevant licenses
