The Appalachian State University campus is currently operating under normal conditions.
New labs, building transfers, PI transfers, and closing labs should send notification through email to the Lab Safety Program (LabSafety@appstate.edu) or by completing this webform. An initial meeting will be held between relevant members of the EHS & EM office and the PI/department to discuss required actions. This discussion could include any or all of the following:
New Labs:
- Suitability of lab space for proposed work
- Lab Safety Program documentation (Lab Safety Plan, chemical/physical/biological SOP’s)
- Applications to relevant safety committees on campus (IBC, IRB, IACUC, etc.)
- Inspection schedule and frequency
Building Transfers:
- Suitability of new lab space for proposed work
- Transfer plan for hazardous materials between lab spaces
- Lab Safety Program documentation updates (documents, door signs, emergency information)
- Final disposition of research materials not being transferred
PI Transfers:
- Transfer or application for relevant licenses
- Lab Safety Program documentation updates (documents, door signs, emergency contact information)
- Inspection schedule and frequency
Closing Labs:
- Final disposition of research materials (biological, chemical, radiological, equipment, etc.)
- Closure of any relevant licenses
