The multi-passenger vehicle driver approval process covers all vans, including rentals, used for university-sanctioned events carrying eight or more total passengers. To be authorized to drive a Multi-Passenger Vehicle, the prospective driver must complete a training program and approval process based on criteria specified in App State Policy 303.13:
For new drivers to the program, please follow the detailed steps below:
Complete in its entirety, and pass, the online Multi-Passenger Vehicle Awareness Course. This course should be completed before submitting the online form.
Print off and have your direct supervisor sign the Multi-Passenger Van Supervisor Approval Form. The signed copy of this form will need to be uploaded electronically in step 3.
Complete the online Multi-Passenger Vehicle Driver Approval Form. Once submitted, Appalachian State University Police will initiate a driver's record check.
Once the driver’s record check is complete, EHS&EM will review the submission and issue a certificate, valid for one year, via email.
Please note that all participants in the Multi-Passenger Vehicle program are required to complete this form annually. It is the responsibility of the driver to submit this form 30 days before expiration to ensure their certification does not lapse.
For existing drivers looking for recertification, please follow the detailed steps below:
